Almost everyone drives. Cross at San Ysidro or Otay Mesa, take the coastal toll road, and you are in Ensenada in about two hours.

Driving down

  • Cross and head south — San Ysidro or Otay Mesa, then Mexico 1D, the coastal toll road through Rosarito. There are three toll booths; keep some pesos or small US bills handy.
  • Buy Mexican auto insurance — This is the one thing you cannot skip. Your US policy does not cover you in Mexico. Day policies are inexpensive and can be bought online before you leave.
  • No vehicle permit — The Baja peninsula is a free zone, so there is no temporary import permit to file, unlike driving into mainland Mexico.
  • Plan the trip back — The northbound border wait is the unpredictable part of the day, not the drive. Check wait times before you head back and avoid Sunday afternoons if you can.

Without your own car

  • Park on the US side — Leave the car in a San Ysidro lot, walk across, and take a bus or shuttle south. It removes both the insurance question and the northbound car queue.
  • By bus — Regular bus service runs Tijuana to Ensenada throughout the day; the trip is around 90 minutes from the Tijuana terminal.
  • Around town — Ensenada's clinic areas and the tourist zone are compact. Taxis are easy and most patients do not need a car once they arrive.
  • By cruise — If you arrive on a port day, pick a clinic near the terminal and book well in advance. Sailing time is not negotiable.
