The short answer
There are two ways onto the island and they suit different treatment plans. Cozumel International Airport (CZM) sits minutes from San Miguel, where most of the 67 clinics Molar City tracks are located. The alternative is the passenger ferry from Playa del Carmen, which runs through the day and takes around 45 minutes. Fly if the island is your base. Take the ferry if you are staying on the Riviera Maya or flew into Cancun.
Route one: fly into Cozumel
This is the simplest version. CZM receives direct service from a number of North American cities as well as domestic connections through Mexico City and Cancun. The airport is a short taxi ride from town, and from there almost everything is within a few minutes.
Choose this route when:
- Your treatment spans several days and you want zero transfers between appointments.
- You are having surgery and do not want a boat crossing in the recovery window.
- You are combining treatment with diving, which is the island's main draw.
Route two: ferry from Playa del Carmen
Ferries leave from the Playa del Carmen pier throughout the day and take roughly 45 minutes. This is the practical route if you flew into Cancun International, which has far more flight options than CZM, or if you are already staying on the mainland.
Choose this route when:
- You found a much better flight into Cancun.
- You are staying on the Riviera Maya with family and coming over only for appointments.
- You want to compare island clinics against mainland ones before committing.
Two cautions. Build slack into the schedule: sea conditions occasionally disrupt sailings, and an appointment that depends on a specific crossing is fragile. And after oral surgery, ask the clinic before booking a crossing home the same day.

When to travel
November through April is the dry season and the busiest stretch, which means more flights and more cruise ships, but also fuller clinic calendars and higher hotel prices. The quieter months are cheaper and easier to book, with hotter and wetter weather during recovery. If you are travelling in the late-summer and autumn hurricane window, buy flexible tickets and leave room in the schedule.
Building a schedule that works
- Book the clinic before the flights. Appointment availability is the harder constraint, not airfare.
- Send records ahead. Recent x-rays and your medication list let the clinic plan rather than start from zero.
- Leave a buffer day before flying home. Especially after surgery, and especially if a ferry is involved.
- Ask about flying after surgery. The clinic should tell you how long to wait. Follow that, not the airline's change fee.
- Confirm the lab timeline. If prosthetic work goes to a mainland lab, your delivery date depends on that, not just on the chair.
Should you be on the island at all?
An honest question worth asking before you book. If your case is complex and you want several specialist opinions, the mainland gives you more to compare: Playa del Carmen and Cancun both have larger clinic counts and no crossing in the schedule. Cozumel makes most sense when you are already coming, when you value the island itself, or when you have found a clinic here you trust. Where to stay
San Miguel puts you walking distance from most clinics, pharmacies and restaurants, which is what you want during treatment. The resort strips north and south of town are more comfortable for a holiday but add a taxi to every appointment. If you are having surgery, stay in town for those days at least.
Getting around the island
Taxis are plentiful in San Miguel and fares within town are short. Scooters and rental cars are popular for exploring the island, but avoid both immediately after any procedure involving anaesthetic or sedation. Ask the clinic when it is safe to drive.
What a treatment week actually looks like
- Day one. Arrive, settle in, no appointments. Jet lag and a first consultation do not mix.
- Day two. Exam, imaging, written plan, decision.
- Days three to five. Treatment, spread out with recovery time between sessions.
- Day six. Review appointment, records handed over, questions answered.
- Day seven. Travel home, with a buffer in case day six runs long.
Compress that into four days and everything still works, until it does not. The buffer is the part people cut and then regret.
