The short answer
All-on-4 replaces a whole upper or lower arch with a fixed bridge anchored on four implants, sometimes six. In Punta Cana the area range is about $8,000 to $12,000 USD per arch, against $20,000 to $30,000 commonly charged in the US. On both arches the difference can be well over $20,000 even after flights and hotels. Full-arch work is also the treatment where the surgeon's experience matters most, so in a resort area with a smaller dental market you need to ask harder questions than you would for a crown.
What a complete quote covers
- Extractions of the remaining teeth and smoothing of the bone.
- Four or six implants from a named brand, with implant cards.
- The temporary fixed bridge, usually fitted on surgery day or within a few days.
- The final bridge after healing: acrylic teeth over a metal framework, or zirconia.
- 3D imaging, sedation, medication and check-ups.
- What happens if an implant fails during healing, in writing.
The single biggest source of confusion is the final bridge. A quote that looks cheap often includes only the temporary, or an acrylic final where another clinic quotes zirconia. Compare like with like.
Prices compared
| Item | Punta Cana (USD) | Typical US (USD) |
|---|---|---|
| All-on-4, one arch | $8,000 – $12,000 | $20,000 – $30,000 |
| All-on-4, both arches | $16,000 – $24,000 | $40,000 – $60,000 |
| Full denture (alternative, per arch) | $400 – $700 | $1,500 – $2,000 |
| Bone graft (per site, if needed) | $300 – $650 | $800 – $3,000 |
Estimates for the area, not a quote. Where a final bridge sits in the range depends mostly on the material.
The timeline
- Remote planning. Send a recent panoramic X-ray or, better, a CBCT scan. A serious clinic will tell you after reviewing it whether you are a candidate and whether grafting is likely.
- Trip one, about a week. Consultation, scan, then surgery day: extractions, implants and the temporary bridge in one long appointment, usually under sedation. The following days are for checks and adjustments.
- Three to six months of healing at home, on a soft diet at first.
- Trip two, about a week. Impressions or scans, a try-in and the final bridge, over two or three appointments.
Surgery day and the days after
Bring a companion. You will be sedated and tired, and you should not be left alone in a hotel room that night. Stay close to the clinic for the first trip rather than at a resort an hour away: Bávaro and Punta Cana village have plenty of mid-range hotels and apartments. Expect swelling and bruising for several days. This is the trip where the beach waits: most surgeons will ask you to stay out of the sea and pools, avoid alcohol and sun, and eat soft food while the wounds close.
Punta Cana or Santo Domingo?
We would rather say this plainly than have you find out later. Punta Cana is a resort area, and its dental market is smaller than the one in Santo Domingo, the capital, which is roughly 2.5 to 3 hours away by road. The capital has more clinics, more specialists and more labs. For routine work, such as crowns, veneers, root canals or a few straightforward implants, a well-run clinic in Bávaro or Punta Cana village is a sensible choice and lets you recover by the sea. For complex surgery, such as major bone grafting, full-arch cases with little bone or zygomatic implants, ask who will operate and where. Some Punta Cana clinics bring in a visiting surgeon or refer patients to Santo Domingo, and it is better to know that before you buy a flight.
Who is a good candidate
All-on-4 is designed for people who have lost most of their teeth in an arch, or whose remaining teeth cannot be saved, and who want fixed teeth rather than a removable denture. The four-implant design angles the back implants to make the most of the available bone, which is why many patients who were told they needed extensive grafting can avoid it. It is not for everyone. Uncontrolled diabetes, heavy smoking, some medications that affect bone and certain heart conditions raise the risk, and a surgeon should ask about all of them before quoting. Be completely open about your medical history and medications in the remote consultation, and send a letter from your own doctor if the clinic asks for one. If you are told you are a candidate without anyone looking at a scan or asking about your health, treat that as a warning.
Questions for the surgeon
- How many full-arch cases have you completed, and can I see some of them?
- Do you operate here every week, or do you travel in for surgery days?
- Who makes the final bridge, and where is the lab?
- What sedation do you use, and who monitors me during surgery?
- If I have little bone, do you do the graft yourself, offer zygomatic implants, or refer me?
- What happens if the temporary breaks after I fly home?
Acrylic or zirconia
An acrylic bridge on a metal framework costs less, is lighter and is easier to repair, but the teeth wear and stain over time and may need replacing after some years. Zirconia is harder, resists stains and usually lasts longer, but costs more and is harder to repair if it chips. Ask for both prices.
Maintenance
A fixed full-arch bridge needs professional cleaning about twice a year, and from time to time the bridge is removed so the implants and screws can be checked. Find out before you commit whether a dentist near home will do that, and bring home your implant cards and a written summary of the case.
Safety and practical sense
The resort strip from Cap Cana to Bávaro is built around tourism, and most visitors move between the airport, their hotel, the beach and a few restaurants without trouble. Read your government's current travel advisory for the Dominican Republic before you book, and follow the ordinary rules: use registered taxis or transport arranged by your hotel or clinic, keep valuables out of sight, do not carry large amounts of cash to an appointment, and drink bottled water, which most clinics and hotels provide anyway.

Frequently asked questions
Do I leave Punta Cana with teeth after surgery?
In most All-on-4 cases you leave with a temporary fixed bridge, fitted on surgery day or within a few days.
How many trips does All-on-4 take?
Usually two trips of about a week each, three to six months apart.
Can I pay per stage?
Many clinics accept payment per stage. Get the schedule and what each payment covers in writing.
Should I go to Santo Domingo instead?
For difficult cases with very little bone, compare quotes and surgeons in Santo Domingo, where the dental market is larger. For standard cases, an experienced surgeon in Punta Cana can be a good option.
